As a Care Coordinator, you will guide older adults and their caregivers through the intake process, providing support and emotional guidance during their journey into care.

The Care Coordinator provides post-intake support to patients by managing scheduling, billing inquiries, and offering empathetic guidance through various channels.

Provides frontline support to a mental health provider network through email and phone. Resolves caseload, technical, matching, payroll, benefits, and patient-related questions; tracks requests through resolution; escalates issues appropriately; and proactively checks in with providers. The role emphasizes organization, follow-through, prioritization, relationship-building, empathy, customer service, and comfort creating processes in an ambiguous startup environment.
