As a Care Coordinator, you will guide older adults and their caregivers through the intake process, providing support and emotional guidance during their journey into care.

The Care Coordinator provides post-intake support to patients by managing scheduling, billing inquiries, and offering empathetic guidance through various channels.

Provides frontline support to a mental health provider network through email and phone. Resolves caseload, technical, matching, payroll, benefits, and patient-related questions; tracks requests through resolution; escalates issues appropriately; and proactively checks in with providers. The role emphasizes organization, follow-through, prioritization, relationship-building, empathy, customer service, and comfort creating processes in an ambiguous startup environment.

Serve as the first point of contact for patients and families through phone, email, and SMS. Respond promptly to inquiries, resolve straightforward requests, triage and route communications, troubleshoot administrative and clinical workflows, and guide patients through their care journey. The role requires empathy, clear communication, organization, reliability, comfort with high-volume communication, and the ability to learn healthcare technology such as EHR and CRM systems.
